This form is to be used to create new or change existing addresses or street names. Examples: Building a new house on an existing, vacant lot that has no address, changing your address for a quicker locate time (emergency response, deliveries, vistors, etc).

Note: Street name change applications have a longer, more indepth process and must be voted on by the City Commission.
