CLAIMS AGAINST THE CITY

Liability claims can be for personal injury, property damage, or for damages or losses that involve a city vehicle/asset. See the options below and choose the correct form to file.

The claimant must submit a claim within 180 days of the incident.

** Please allow at least 45 days for the Risk liability team to investigate your claim.

Once the City receives the completed form and details, the claim is then investigated to determine liability. If you need to include additional materials after you submit your form, please reach out to Risk Management with your name and details. Please include any estimates for repair and photographs (electronic preferred).
